What is Support At Home ?

The Australian Governmentโ€™s Support at Home Program helps you to live independently in your own home for as long as possible. The program provides funding towards a range of care, support services and care management tailored to meet your individual needs. A professional assessor will work with you to determine if Support at Home is the right service for you, what types and levels of support you require, and when you might need them.

There are 8support levels within the Support at Home Program, each offering different amounts of funding based on the level of care and services you need. The funds are paid to the approved provider you choose, who will manage the funds and deliver care management, support services, advice, and coordination on your behalf. These funds help cover the cost of the services you receive, but they are not paid directly to you. Higher support levels provide access to more intensive or frequent care, depending on your individual needs.

The Home Care Package Level 1 is for individuals needing basic assistance with daily tasks.ย

Frequently Asked Questions

You may be eligible for Support at Home if you are:

  • an older person who needs coordinated services to help you stay living safely and independently in your home
  • a younger person with a disability, dementia or other special care needs that are not being met through other specialist services

There are no minimum age requirements or residency restrictions to access government-funded aged care services. However, Support at Home is not intended for:

  • visitors to Australia
  • people requiring temporary or short-term care
What Steps need to be taken to get Support At Home?

To check your eligibility for Support at Home or other in-home aged care services, visit the My Aged Care website or call their contact centre at 1800 200 422.

The My Aged Care team will ask you a few questions to determine if you need an assessment by an Aged Care Assessment Team (ACAT). If an ACAT assessment is required, they will evaluate your needs and confirm your eligibility for Support at Home, including the appropriate level and type of care you may require.

Eligibility for Support At Home +

To meet your eligibility for Support at Home you need to be 65 years and older, Aboriginal or Torres Strait Islander and aged 50 and above, or 50 and older and live in a rural remote area, additionally you must have residence in Australia. Support at Home will help you live independently in your own home for as long as possible. For more information click the button below to speak to our team.

What Steps need to be taken to get Support At Home? +

To meet your eligibility for Support at Home, contact My Aged Care service. They will assess whether you're eligible and determine if you could benefit from a face-to-face assessment with an Aged Care Assessment Team (ACAT). If the ACAT determines that Support at Home suits your needs, the ACAT will determine which Support at Home level best addresses your care needs. You can find out more about the My Aged Care service assessment process and the ACAT face-to-face assessment process by visiting My Aged Care website. The ACAT assessment takes into account your current and future needs, including the appropriateness of remaining at home or moving to residential care. Following your ACAT assessment, you will be issued with a referral code. This referral code will allow you to choose an approved provider (such as us!). Rose Aged Care will then provide and manage personalized care for you.
